Pills, Pot and Powdery Substance – Practical Considerations for MFMs in Addressing Perinatal Substance Use

Course Description: 
This case-based session is designed to provide MFMs with practical tools to assess and manage pregnancies complicated by drug use. We will review evidence-based approaches for screening, diagnosing and optimizing care for women with substance use disorders. The session will review impact of opioid and methamphetamine epidemics on pregnant and parenting women. There will be emphasis on practical steps for peripartum pain management and drug testing, including utility and pitfalls of urine drug testing and biological testing of the newborn. Given the increasing legalization of marijuana, a portion of the session will be dedicated to reviewing the data on marijuana use in pregnancy and breastfeeding with emphasis on practical tools for counseling and treatment during pregnancy.

Learning Objectives:
  • Describe the management of pregnant and postpartum women with substance use disorders (stimulant and opioid use disorder) including evidence-based counseling regarding pharmacotherapy and behavioral treatments, post-delivery pain management consideration and neonatal abstinence/neonatal opioid withdrawal syndrome.
  • Review the current data regarding marijuana use in pregnancy and counseling recommendations including use in pregnancy and breastfeeding. 
  • Describe utilization and limitations of drug testing during pregnancy and newborn testing including state specific laws and provider obligations.
